State of Ohio BMV Requirement

State regulations require physical, signed documentation for enrollment. We cannot accept fully online registrations at this time. Please download the appropriate forms below, print them, and complete them with physical signatures.

Submission Instructions

  • 1.Print the downloaded PDF document.
  • 2.Fill out all required fields entirely in pen. Ensure all necessary parent/guardian signatures are included where indicated.
  • 3.Secure your deposit payment (Check or Money Order).
  • 4.Mail the completed forms and deposit to our office, or drop them off in person during our scheduled office hours.
